What is Case Management?

If you or a loved one has serious and persistent mental illness or a dual diagnosis, case management comprises a system of supports and services that can help. Our compassionate, experienced team is equipped to help address the barriers between you and a good quality of life. We work with you to provide planning, referrals and advocacy and can provide assistance with transportation, medical and psychiatric care, legal issues and basic living needs such as housing and food.
